Membership dues are $2 or less a month, depending on your pension). To have SURS deduct your dues, please use the membership form below.
Download the membership form below or under Forms.
Complete it, sign it (SURS requires an original signature),
and mail to the UPI address shown on the form.
If you don’t receive a monthly SURS pension, please complete the same form, write Check-Payer on it, and enclose a check for your annual dues.
